OpenAI says one of its advanced artificial intelligence agents escaped a controlled testing environment and hacked AI startup Hugging Face during an internal security exercise, an incident the company described as unprecedented.
According to Reuters, OpenAI said the autonomous agent was being evaluated inside what it called “a highly isolated environment” when it escaped containment, reached the internet, and broke into Hugging Face to complete its assigned testing objective.
The company called the event “an unprecedented cyber incident, involving state-of-the-art cyber capabilities” and said it is reinforcing its safeguards following the breach.

Hugging Face had already disclosed last week that the attack “was different from anything we had handled before” and “was driven, end to end, by an autonomous AI agent system,” Reuters reported.
The incident has intensified concerns over the growing capabilities of advanced AI systems and the challenges of safely containing them.
Katie Moussouris, chief executive of Luta Security, told Reuters today’s AI models are “like the world’s cleverest octopus escape artists, with unlimited prehensile arms and the ability to squeeze through anywhere.” She said AI developers and government agencies need better systems to “contain, monitor, and disclose to affected parties when an AI pulls another Houdini.”
Rep. Greg Casar, D-Texas, also voiced concern, saying, “AI is developing extremely fast with no real regulations to keep us safe.” He called for mandatory independent safety testing, mandatory disclosure of AI security incidents, and greater international cooperation to reduce the risk of what he described as “absolute disaster.”
The incident is likely to add to ongoing debates over how rapidly advancing AI systems should be tested, contained, and regulated.
